Oxfordshire County Council, in collaboration with the NHS and local partners, are developing a plan for services which support people living with dementia and their families/carers.


We want to understand what works well for people now and what could be better.


What you tell us will help to develop a plan for the future that is informed by people with lived experience of dementia, their carers and the health and care professionals looking after them.


Your response to the survey is important to us and will help us to identify gaps and challenges which will be addressed in the new dementia strategy.


We would like to invite you to give your views by completing our surveys. These typically take around 10 minutes to complete, depending on the detail that you wish to add.


Thank you for taking the time to give your views and experiences about living with dementia and/or caring for people living with dementia and how you find your way around the support and information available.
