Oxfordshire County Council’s Live Well at Home (LWAH) Framework will expire in March 2027. The council is designing a new model for the new framework and would like to seek feedback from providers on this. The survey below has some questions which will help with this feedback.
The new framework will be an integrated framework and will be used to source home care for individuals that have met the Care Act eligibility criteria of Continuing Health Care (CHC) criteria.
