Oxfordshire County Council (the Council) is looking for a Service Provider for a supplementary NHS Health Check programme. Currently the NHS Health Check programme is delivered by GP Practices only throughout the county.
The Council has issued this invitation to have a conversation with suppliers in the market to help draft a future supplementary servicemodel for the provision of the NHS Health Check programme in the County from 2022/23.
The aim is to use the markets insight, expertise, comments, and observations to help the Council develop a final alternative service model that is in step with, and is as attractive as possible, to your organisation. This process will also provide the Council with content for the development of key documents within any draft Contract that would be published should the Council decide to issue an Invitation to Tender (ITT) next year (i.e., a draft Specification, Monitoring/Review and Finance Schedules.
All responses to this questionnaire will be confidential. Please note that you can chose to answer some of the questions, or all of the questions, as they are not mandatory.
