Consultation has concluded
We are contacting people receiving care and support through our Live Well at Home Framework. The framework enables the council to provide support for people to live as independently as possible living in their own home or Extra Care Housing.
The support that you or your loved one receives is provided by a company that Oxfordshire County Council contracts with to make sure you receive a high standard of care.
The council is reviewing the Live Well at Home Framework and is seeking the views of those individuals and families who have benefitted from this service. It is important for us to understand what works well for people using these services in Oxfordshire and what could be better.
We would be grateful if you or your family could complete the short questionnaire below and submit it by no later than 4pm on 18th December 2025.
When you submit this form, it will not automatically collect your details like name and email address unless you provide it yourself.
