Combined consent form and payment for involvement form

This combined online consent and payment for involvement claim form must be completed in full, signed (online) and dated, before signed-up participants can join the travel and transport sounding board on Saturday 25 March 2023 .

Young people will be unable to participate unless they have provided a completed consent form, 

To guarantee your place, please complete and submit this form as soon as possible, and by 11.59 pm on Wednesday 22 March at the latest.

If you would like to claim for travel expenses, an online link to the claim form will be sent after the event.
